PCN Social Prescribing Lead Maternity Cover



12 month contract (with possibility of continuation)



Hours

: 22.5 hours a week; flexing to meet the needs of the business as agreed within the Red Zebra Management Team.?

Time off in Lieu is given for any hours over contracted hours.

Job Purpose:

To take a lead in managing Red Zebra's Herne Bay PCN Adult Social Prescribing contract: enhancing referrals and external organisation participation, liaising with partner and stakeholders to improve the service and the overall health and wellbeing of the communities we serve.

MAIN DUTIES & RESPONSIBILITIES

?

Hold a case load of ten patients- this can be related to a specialism as allocated by need/ RZ management. Manage, train and support Social Prescribing Link Workers (5 team members, 3 p/t, 2 f/t), setting KPI's, conducting 1-2-1s every quarter, observations (x2 annually per SPLW) and managing absence and holiday records in tandem with HR. Work closely with partners for the PCN to ensure consistency across the project. Monthly meetings with PCN leads to discuss progress, direction and support of project. Support and participate in accessing SPLW's with improvement and refinement of practice including work place capability assessment and any disciplinary measures required. Deliver training and presentations to external organisations to increase awareness of social prescribing Write progress and evaluation reports as guided by contract (quarterly) Monitor referrals and referral process ensuring a timely, responsive, professional and well-rounded service Monitor and evaluate any outreach opportunities undertaken by SPLW team. Evaluate reports to ensure population reach is varied and identify improvements required on a local (SP) and project level. Gather national and local good practice on Social Prescribing services to develop links, relationships and best practice support with other local and regional services alongside ICB & CYP lead. Examine processes, looking at where improvements can be made Liaise with health and social care and community based professionals to establish effective working relationships Manage information governance and data protection protocols to ensure the security of all social prescribing client data for the PCN. Ensure staff training is tracked, up to date and relevant, access case by case basis for RZ sponsored training- identify training needs and nurture specialisms where and when relevant to PCN aims. Manage the attendance of the CHOCs/MDTs by SPLW team Manage volunteer programme and develop this service to support the project as/ if required. Work alongside management team to identify strategic development of the project Organise team support in the way of team meetings, case load reviews, complex case reviews and peer support, working in tandem with ICB and CYP Lead. Organise training opportunities from outside organisations on a 1-2-1 or group basis for staff development, working in tandem with ICB and CYP lead. Manage recruitment; shortlisting and interviewing candidates alongside administrative support of advertising, candidate support and training timetable alongside the Social Prescribing Administrator, Management Team and other project Leads. Educate staff on public health developments, and best practices to enhance service delivery. Conduct training sessions for new employees, focusing on essential skills and knowledge required for their roles. Work alongside the Senior Social Prescriber to facilitate effective communication among team members to ensure continuity of operations. Address any issues or conflicts that arise promptly and professionally. Maintain accurate records of activities, including staff attendance, incidents, and support management team in the development and implementation of PRSIF Assist in the implementation of health initiatives aimed at improving community wellbeing.

ADDITIONAL RESPONSIBILITIES



Work closely with Community Service Manager and Management Team to develop linked projects and referral pathways between Social Prescribing projects. Support with social media output across the charity (Facebook, Instagram, LinkedIn, etc) Set up and coordinate special events and outreach events with the support of the PCN in various locations Ensure that the Social Prescribing service reflects the vision, mission and values of Red Zebra Support the running of the main office (Clover House) with volunteers, phone calls and office management. Support with updating policies and procedures as and when necessary to reflect changing legislation. Recognise people's needs for different methods of communication and respond accordingly Adhere to Red Zebra's policies and procedures at all times including Equality and Diversity, Health and Safety, Confidentiality, Lone Working and Safeguarding Assume responsibility for producing own correspondence, performing and maintaining administrative tasks Undertake any other additional tasks as reasonably deemed by the Board of Trustees and Management Team.
This role is ideal for individuals who are passionate about public health and possess the skills necessary to lead a dedicated team towards achieving excellence in service delivery.
