We're looking for a warm, proactive, and highly organised

HIU Care Coordinator

to join our dedicated team. This is your chance to be at the heart of a service that transforms lives by supporting individuals who frequently use urgent and emergency care services.

What You'll Be Doing:



As our HIU Care Coordinator, you'll be the vital link between patients, healthcare professionals, and community services. You'll:

Identify and track individuals who need extra support using a dashboard. Coordinate and lead multi-disciplinary team (MDT) meetings. Liaise with GPs, hospitals, social care, and voluntary sector partners. Develop tailored care plans and ensure follow-through on referrals. Be the friendly, knowledgeable point of contact for all things HIU-related.

What We're Looking For:



We're seeking someone who is:

A natural communicator with excellent interpersonal and organisational skills. Experienced in working with vulnerable individuals or in a healthcare/social care setting. Confident using IT systems and managing sensitive information. Passionate about reducing health inequalities and improving patient outcomes.
Bonus points if you have experience in mental health, substance misuse, or housing support--but we're more interested in your attitude, empathy, and drive to make a difference.
