Community Care Worker

Duties:
- Provide mentoring and support to individuals in the community
- Assist with daily living activities, such as meal preparation, personal hygiene, and medication reminders
- Help individuals develop and maintain social connections and participate in community activities
- Collaborate with healthcare professionals and family members to ensure the well-being of individuals
- Monitor and document progress and any changes in behavior or health status
- Provide emotional support and encouragement to individuals facing challenges

Experience:
- Previous experience in a caregiving or mentoring role is preferred
- Knowledge of community resources and services
- Strong communication and interpersonal skills
- Compassionate and patient demeanor
- Ability to work independently and as part of a team
- High school diploma or equivalent
