Part Time / 24 and 30 Hours Per Week / Fixed Term for 12 months



Would you like to be part of a team who aim to make the transition from hospital to home easier for the residents of Leicester City?



We're currently looking to hire a number of Reablement Assistants who will play an integral role in supporting the residents of Leicester City who have recently left/are due to leave hospital after ill-health. The Reablement Service are a small dedicated team, whose main goal is to enrich the lives of those returning home to ensure they maintain their independence and healthy living in a safe manner within the comfort of their own home.



As a Reablement Assistant you'll be required to work alone, mostly, travelling in your own car within Leicester City to the hospital and residents own homes however, at times your caseloads will link with other Reablement staff and other professionals. You'll work alongside other members of the Reablement team (Assistants, Officers, Coordinators) to provide short-term support for people within their own home. You'll be expected to link in with other professionals through face-to-face conversation and telephone correspondents (Doctors/GPs, Physios, Therapist and Nursing staff) for a multidisciplinary and joined up Home First offer. In addition, you'll attend supervision and meeting periods within The Neville Centre.



We have positions available on a fixed term contract of 12 months. The working patterns for these roles are 24 and 30 hours per week. These hours will be based on a flexible rota working mornings and evenings 7 days a week, this includes every weekend, bank holidays and other public holidays. Salary shown on advert is based on 30 hours.



What you'll be doing





As a Reablement Assistant you'll support people on their Reablement/ICRS journey to independent living. Your daily tasks will be care based in nature, including but not limited to supporting individuals with their:


personal care (washing/dressing)

mobility

transfers

medication management.


In addition, you'll be expected to accurately document and record records of care provided, reporting levels of functioning, improvements, and concerns. This includes contributing to a daily written record of the persons progress ensuring that all records are accurate and a true reflection of the intervention.



All tasks are to be completed with upmost confidence and professionalism to be able to meet the expectations of a CQC regulated service.



What you'll need





The ideal candidate will be a confident care giver with experience of working within a health or social care setting, specifically those that include the provision of care to adults and older people with complex needs (for example: a physical disability, a learning difficulty or a mental health diagnosis).



As a lot of this work involves lone working, you'll need to be disciplined with yourself, have excellent organisation skills in order to prioritise your workload and be able to work under pressure in a busy and often difficult job role.



In addition, you'll have great interpersonal skills. You'll be able to communicate effectively to a wide range of people from varied and diverse backgrounds, ethnicities, and educations in both verbal and written contexts. As a confident person you'll be able to match your approach to the needs of each individual as they are encountered with a flexible and no-nonsense approach.



Overall, you'll be an eager candidate who is keen to learn and ready to take on new skills within a short period of time in order to consistently perform highly alongside their team/service to offer a service to individuals that is safe, timely and compassionate.



Due to the nature of the role you'll need a full driving licence. This post is also subject to an Enhanced DBS check.
