service provide short term reablement care and support to help people stay independent within their own home.
As a self-motivated, caring, and passionate person who shares the same care and dedication as we do, you will join our teams across the county as a
Reablement Support Worker
(RSW).

you will receive a thorough induction, in-depth training and full support from your team to grow your skills and development.
You will also have the option to complete a
fully funded
level 2 qualification
inHealth and Social Careto advance your career in Health and Social Care.
In this role you will:
create and maintain positive relationships with customers and family carers
promote health, wellbeing and independence to the customer
develop the independence of customers through care plans / short term enablement plans
provide general advice and guidance to customers to help them to make informed choices
at times, assist customers with dignified and intimate personal care needs
engage with customers, family carers and others
promote alternative ways to meet and maintain their long-term needs
promote a range of services to help people live independently and safely at home
track customers' progress against care plans
complete records for the ongoing review process.
