Hospital at Homeis an exciting service that delivers acute-level care in the comfort of a patient's own home. Working as part of a multi-disciplinary team, you will play a vital role in reducing unnecessary hospital admissions, supporting early discharges, and enhancing patient outcomes through personalised care.
Based across West Essex, including Harlow, Epping, and Uttlesford, you'll work in a fast-paced, collaborative environment that puts patients and families at the heart of everything we do.

Main duties of the job
Provide senior clinical and operational oversight to ensure safe, effective, and continuous Hospital at Home service delivery.
Lead the coordination, triage, and timely admission of referrals, ensuring patients access the most appropriate care pathway.
Undertake a weekly in-reach rotation at Princess Alexandra Hospital to support early discharge planning, multidisciplinary working, and identification of patients for community pathways under the "Home First" ethos.
Work with acute, community, and system partners to prevent unnecessary admissions and support coordinated, safe discharges.
Oversee day-to-day operational issues, escalate as needed, and manage caseload flow across the service.
Monitor and maintain compliance with staff supervision, appraisals, and mandatory training.
Lead and participate in clinical audits, reflective practice, and learning from incidents to maintain high standards of care, governance, and continuous improvement.
Represent and promote the service across partner organisations, contributing to service development, pathway improvement, and NHSE recommendations.
