We are seeking a compassionate, reliable, and dedicated
Support Worker
to join our team, providing person-centred care and support to
vulnerable adults
. The successful candidate will play a key role in promoting independence, dignity, and wellbeing, supporting individuals to maintain stable accommodation and develop essential life skills.

The Support Worker will work closely with residents, support plans, and partner agencies to deliver tailored, outcome-focused support.
Key Responsibilities
Provide personalised support to vulnerable adults in line with individual support plans
Assist residents with daily living activities, including personal care, hygiene, budgeting, and maintaining accommodation
Promote emotional wellbeing, independence, and social inclusion through regular engagement and structured support sessions
Support residents to access healthcare, benefits, education, employment, and community services
Maintain accurate, timely, and compliant records of support delivered, progress, and outcomes
Identify and manage risks, escalating safeguarding concerns in line with organisational policies
Work collaboratively with Local Authorities, healthcare professionals, probation services, and other partner agencies
Safely accompany or transport residents to appointments or activities where required
Use IT systems confidently to record support sessions, update case notes, and communicate with colleagues
Ensure health, safety, and property standards are maintained at all times
Essential Skills & Experience
Experience working with vulnerable adults in supported housing, homelessness, mental health, substance misuse, or social care settings (desirable)
Strong understanding of person-centred, trauma-informed support practices
Ability to develop, follow, and review individual support plans
Excellent communication skills, with the ability to build professional, respectful relationships
Confident in maintaining clear, accurate written records and using IT systems
Ability to work independently, manage time effectively, and remain calm under pressure
Compassionate, patient, organised, and professional at all times
Desirable Criteria
Full UK driving licence with a clean driving record
Knowledge of safeguarding adults, risk assessment, and support planning
Experience working with multi-agency professionals
Understanding of Housing Benefit / Universal Credit within supported accommodation (advantageous)
