Job Summary
We are looking to recruit an enthusiastic and dynamic Independent Prescribing Pharmacist to join our growing multi-disciplinary primary care team. You will work alongside our multi-disciplinary clinical team of GPs, Nurse Practitioner, Pharmacists, Pharmacy Technician and Nurses to deliver high quality patient care.
The contract is offered on a full-time basis of 37.5 hours per week, worked over 4-5 days. Protected learning time, CPD, and support from a dedicated GP supervisor will be provided to help you flourish within the role.

Main Duties of the Job
With support from your dedicated GP supervisor, you will provide care for our registered patients. You will demonstrate safe, clinical decision making and expert care, including assessment and diagnostic skills, for patients within the service. You will demonstrate critical thinking in the decision making process. You will work collaboratively with the healthcare team to meet the needs of the patients and support the delivery of policy and procedures. You will provide a holistic and clinical service, with support and supervision from GPs, implementing agreed management plans and following approved protocols as appropriate.
The right candidate will also have the opportunity to take leadership of chronic disease management within the practice, running dedicated clinics to help our patients to manage their condition.
Essential Criteria
GPHC registered for at least two years
IPP Qualification
Evidence of relevant CPD and work experience
An interest in chronic disease management
Excellent time management
Preferred Criteria
Specialism/special interest in a chronic disease area such as Diabetes, Asthma, COPD or Hypertension.
Disclosure and Barring Service Check
