We are looking for an enthusiastic and committed registered mental health nurse to join our Intensive Support Service.
The team works intensively with client's for up to eight weeks to reduce risk of admission and to support post discharge from mental health units. The team provides a liaison role and is in regular contact with inpatient services to facilitate discharge.
The successful candidate will be providing home visits to both older adults with age-related complex mental health needs and to adults of any age with dementia in which to enable observations and interventions.
Candidates must have the ability to travel independently between sites or have access to reliable transportation for travel between sites.
The role of Community Nurse is to work in partnership with service users in the designated care group with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives. They will demonstrate an understanding of clinical interventions relevant to the care group to include education, care plans and discharge planning, working closely with carers, families and partner agencies to facilitate this. The post holder will work at all times to promote the safety and the well being of service users and their families/carers. The post holder will assess, plan and implement care, and provide specialist nursing advice and carry out specialist nursing procedures. They will provide clinical supervision to staff and students and act as a mentor for student nurses.

Responsibility for managing an allocated workload, providing agreed intensive levels of support and interventions and establishing priorities within it.
To ensure that the care plan is fully implemented with the service user in accordance with service specifications including liaising with and providing information to other agencies involved in the care of the service user.
Monitor the quality of care delivered and service user and carer satisfaction with the service.
Assist in reviews of service user in the community on a systematic basis to ensure the continued need and appropriateness of services provided by the Trust and maintain review records.
Adhere to, and where appropriate, ensure the implementation of relevant legislation, Department and Trust policies and procedures
Maintain accurate and up-to-date records as required by the Trust.
Participate in a formal programme of supervision.
