Are you passionate about supporting people to regain their independence after illness, injury, or hospital discharge? As an

Intermediate Care Co-ordinator

, you will play a vital role in supporting individuals, primarily older adults, transition safely and confidently back to independent living.

In this rewarding role, you will work closely with individuals and a multidisciplinary team to assess needs, develop tailored reablement plans, and coordinate timely, person-centred support. Your efforts will ensure that the right support is delivered at the right time, maximising recovery and preventing unnecessary readmission to hospital or long-term care.

What we are looking for:



Strong communication and organisational skills Compassionate, solution-focused, and calm under pressure Ability to work collaboratively across health and social care settings Experience in care coordination, health or social care (preferred, but not essential if you have the right mindset and transferable skills)

The role includes:



Working within an intermediate care multi-disciplinary Team partnered with Hywel Dda Health Board Triaging new referrals and helping prevent unnecessary hospital admissions Supporting individuals with goal-setting and rehabilitation planning Working closely with individuals and professionals to promote independence and recovery Contributing to a collaborative, person-centred approach to care
