About the Role




As a Keyworker, you will provide personalised, community-based support to children and young people aged 0-25 with autism and/or learning disabilities who are at risk of mental health hospital admission or are currently in inpatient settings. Your role will be pivotal in:
Building strong, trusting relationships with children, young people, and their families/carers. Acting as a key point of contact to coordinate care and support. Supporting the implementation of personalised plans. Working across services to remove barriers and improve outcomes. Participating in Care, Education, and Treatment Reviews (CETRs).

What You'll Do




Provide direct support to children and young people on the

Dynamic Support Register (DSR)

. Collaborate with multi-disciplinary teams across health, education, and social care. Contribute to service development and evaluation through data collection and feedback. Attend and contribute to national and regional Keyworker Community of Practice events.

Travel Requirement




This role involves significant travel across the Dorset Council area. A suitable vehicle must be available on most working days.

What You'll Need




experience working with children and young people with complex needs strong communication and relationship-building skills ability to work flexibly across services and systems enhanced DBS check (adults and children).
