To support a Client to live their life in the way that they choose and in their best interests and be crucial communication and role at the end of the DAY shift and beginning of the NIGHT shift in the Home.
Hours of Work:
3pm - 9pm on a 4 week rolling rota:
WEEK 1 - Friday and Sunday
WEEK 2 - Thursday and Friday
WEEK 3 - Friday and Sunday
WEEK 4 - Thursday and Sunday
Reporting to:
Home Manager/Deputy Manager/ Team Leader
Required:
Social Care Wales Registration (you will be assisted if you are not)
ROLE SNAPSHOT:
Brief handover with Team Leader
Tea & coffee round, check client's orders for supper and who wants trays in rooms
Prompt/ support Clients to go to the toilet if appropriate
Responsible for the clients in rooms:
Serve supper trays, ensuring that they are encouraged or assisted to eat and remove trays once finished.
Report back to Care team if any delays or issues
Some Clients may choose to sit in the Garden Room and watch TV after supper so assist with moving Clients out of the dining room if needed.
Work with the Care team to assist clients
if
they choose to go to their bedrooms, support them to change into their night clothes, pressure relief, pad changes, oral care, help clients have a wash and assist with personal care if needed.
Carry phone till 9pm
Tea trolley (ensure drinks are decaffeinated where indicated) and collect dishes etc. from rooms.
When done, assist the Clients who want to go to their bedrooms
Report any issues or concerns to NCL.
Job Responsibilities and Duties:
Monitor all residents whilst on shift, taking appropriate action when needed.
Support each resident as agreed and designated in their personal plan.
Sensitively assist and support residents in all aspects of personal hygiene as needed during the shift
Assist and support residents in dressing/undressing and bed making as required.
Sensitively assist and support residents in all aspects of personal hygiene including bathing and/or showering.
Preparation and serving of drinks as required and requested if appropriate.
Keep all communal areas clean and tidy.
Attend to any medication needs as directed, including collecting any samples.
Report any concerns or issues regarding the residents to the A Team as appropriate, including the reporting of any complaints.
Carry out all necessary actions in case of an emergency, including calling the relevant service, ambulance, fire, or police, and inform Registered Manager or other responsible person.
Be aware of health and safety issues when working with the residents and work within the Health and Safety at Work Act during hours of employment.
Communicate with other care professionals when appropriate regarding the delivery of care and support to the resident/s.
To report any safeguarding concerns, immediately, to the appropriate person
Maintain an attractive and stimulating environment and assist/encourage residents to join in activities
Carers are expected to help in activity sessions by encouraging residents to attend and taking instructions from the person putting on the activity to ensure the smooth running of the session.
Assist in serving of main meals in the dining room ensuring residents have sufficient food and drinks. Assist in collecting dishes etc.
Welcome and provide hospitality to visitors.
Communicate with other care professionals when appropriate regarding the delivery of care to the resident/s.

Knowledge and Training:
An understanding of present philosophies in the care of the elderly and supporting people in a residential home and the legislation governing this area of care.
A commitment to training relevant to providing regulated activities in a residential home and personal development of knowledge and skills in this area.
Agree to undertake statutory training to meet requirements under RISCA and the SSWB Act, including NVQ 2 in Health & Social Care.
Work according to the Social Care Wales Code of Professional Conduct
