Job Title: Lead Nurse/Non-Medical Prescriber - Band 7

Responsible To: Clinical Lead

As Lead Nurse/NMP you will have responsibility of leading and managing the nursing team and providing substance use detoxification to clients admitting and in residence of our specialist addictions residential detoxification and rehabilitation service.

OVERVIEW:



As Lead Nurse/NMP and member of our multi-disciplinary team the post holder is responsible for the provision of high-quality nursing care and clinical services to patients in relation to our residential addictions' detoxification and rehabilitation service. At Abbeycare the post holder will be expected to work in partnership with other team members to provide and contribute to the development of our service and respond to the needs of all patients who will present with complex mental and physical health needs.

MAIN DUTIES & RESPONSIBILITIES



ADMISSIONS:



1. Abbeycare Admissions team will provide you as Lead Nurse/NMP with a medical summary report in an agreed format for all prospective admissions to our substance use detoxification rehabilitation service subject to patient and GP consent. Should the client decline to provide a medical report, or the clinic be unable to access the medical report the Lead Nurse/NMP will decide based on the identified assessment needs and complexity of the case whether to approve admission. If unsure or require a second opinion advice will be provided by our Clinical Lead.

2. On occasions more complex presentations can be deferred to our Clinical Lead to approve for admission and take responsibility for the clinical management plan.

3. Carry out a health care and substance use assessment for each patient at point of admission to Abbeycare.

4. Responsibility for assessing the suitability of patients from a health perspective to be admitted to the Service. Should you feel that referral to our Consultant Psychiatrist or Clinical Lead (both of whom have specialist interest in addictions) may be required you will coordinate the appropriate further medical interventions. If alternative external arrangements are required, you will advise on appropriate onward referral.

PRESCRIBING:



1. As prescribing Lead Nurse/NMP you will tailor a detoxification protocol for each patient and will agree the protocol with the patient. You will spend sufficient time with the patient to ensure that they understand the detoxification regime and agree to it.

2. Provide a prescription that is acceptable for Pharmacy use for detoxification and any other medication required by the patient during their residence at Abbeycare.

3. Any PRN medication will be prescribed by you with clear, written guidance to colleagues regarding the administration of that medication including dosage, frequency, and storage.

CLINICAL/TREATMENT PLANNING:



1. Provide Abbeycare with a written assessment report, in the agreed format, upon completion of the initial patient consultation. This will include an ongoing clinical management plan identifying all supporting or additional medical input required for each patient through the detoxification and rehabilitation process such as harm reduction interventions, additional nursing, dietician, or OT input etc.

2. Coordinate or undertake the ongoing assessment of health care needs including developing, implementing, and evaluating action plans whilst involving the patient to promote the recovery process empowering the patient to take responsibility for their health and wellbeing.

3. Use clinical judgement in the provision of care to enable people to improve, maintain, or recover health, to cope with health problems, and to achieve the best possible quality of life, whatever their current condition is when they enter our care.

4. Collect samples, analyse, and record results of urine/oral fluid/blood samples, following specified protocols. You will coordinate sample collection and processing using the Abbeycare account with the local biochemistry.

5. Instruct the use of PRN assessment tools such as CIWA-Ar / CIWA-B / SOWS etc.

6. It will be the responsibility of Abbeycare management to ensure the PRN assessment tools are always implemented to assess PRN detoxification requirements.

7. Ensure appropriate levels of visual and physical observations are carried out and recorded through detoxification.

8. Ensure the clinical team carry out risk assessments for mobility, self-harm, suicide when required and these are monitored, reviewed.

9. Ensure safeguarding risks are responded to appropriately by the nursing team using clearly defined protocols.

10. Carry out or instruct the nursing team to carry out appropriate substance screening with baseline admission screening and appropriate screening through the treatment period.

11. Provide patients with a range of harm minimisation information and signposting for, advice and support such as safer injecting (needle exchange locations), safer drug use, safer sex and other appropriate health and social care issues.

12. Provide or instruct nursing staff to provide take home naloxone training to opiate detoxification patients and their families.

13. Assess and provide written consent for patients to receive home medication for symptomatic relief during their stay at Abbeycare.

14. Carry out all clinical practice following best practice in relation to infection control.

15. Assess patient's need for services and refer to other healthcare services where appropriate.

16. Promote shared care and partnership working.

17. Ensure clear and concise reports and accurate nursing records are maintained in accordance with the Policies and Procedures and NMC record keeping.

18. Provide written consent for patients to receive holistic therapies, attend the gym or swimming during their residence at Abbeycare provided such activities are considered by you to be appropriate and safe for the patient. It is however agreed that the holistic therapist will conduct their own specific assessment prior to holistic therapies.

19. Provide information or assist in clinical practice audits such as medication etc.

20. Ensure appropriate clinical equipment is available for the role and the service ensuring appropriate equipment servicing.

21. Provide information or data for the reporting of medication and incident notifications to the Care Inspectorate.

22. Ensure the nursing team feel supported and motivated in their practice and work to best practice guidelines.

23. Deliver line management supervision to the nursing team.

24. Provide discharge medication from the service, subject to risk assessment, when required.

25. Provide clinical discharge information to the appropriate internal and external clinical care providers.

26. Support and contribute to the development of organisational clinical protocols and policies.

27. Initiate and contribute to the development and realisation of clinical/research audit projects.

28. Understand and adhere to current legislation e.g., Supporting & Protecting Adults at risk.

29. Keep senior staff informed of all relevant circumstances and report complaints relating to the service to the Registered Manager.

TRAINING:



1. If required assist in updated detoxification protocol training to the nursing and recovery practitioner team.

2. Responsibility to engage in and deliver clinical supervision regularly to meet Care Inspectorate & NMC requirements and provide Abbeycare with documented details.

3. Assist in the induction of new staff, in terms of clinical protocols.

4. Act as a role model and assist colleagues to develop their professional role and skills.

5. Assist in the delivery of formal and informal teaching sessions, and support others in providing education for patients, relatives, and colleagues.

6. Maintain and update skills and knowledge in substance use prescribing and nursing care.

ON CALL:



1. Coordinate and provide on call telephone support to Abbeycare regarding detoxification and healthcare issues on a fortnightly basis.

2. In emergencies should the on-call team who provide 24/7 on call support be unable to provide cover appropriate alternative cover will be provided.

GOVERNANCE:



1. Responsibility to ensure Abbeycare that NMC registration is up to date.

2. Participate in regular clinical governance meetings and clinical working groups with the other members of the clinical and management team to develop individual and service practice.

3. Engage and utilise in your own line management supervision to reflect and develop practice.

4. Responsibility to ensure that your clinical training is up to date and appropriate for the role with records available to Abbeycare Scotland.

5. Medical practice insurance will be provided through Abbeycare Insurance policy cover.

Pay: 55,000.00-57,000.00 per year
