Lead Practice Nurse

to join our growing service at Westgate Surgery. This is a Permanent part-time job (27.5 hours per week). The full-time equivalent for us is 40 hours per week.


This is a key leadership role, responsible for delivering safe, effective, and person-centred care across our diverse patient population. You'll lead a skilled nursing team, working closely with GPs and the Practice Manager to support day-to-day clinical services while championing clinical excellence, patient safety, and staff development. They will look to develop new ways of working which improve quality of care for patients, with a focus on the pro-active management of long-term conditions, in line with our mission to improve lives. We're particularly keen to hear from candidates with experience and/or qualifications in the management of long-term conditions such as Diabetes, COPD and asthma, who can bring specialist knowledge and contribute to the continued development of our chronic disease services.


The post holder will work to invest in and strengthen our nursing team whilst working to develop new models of care. They will play a leading role in our approach to Personalised Planned Care, the management of chronic conditions; and work to enable QOF and other clinical quality related achievements. They will offer senior clinical support and leadership to our nursing team and work to promote a high performing, sustainable and engaged culture across the clinical workforce.


Working with the People Team they will contribute to workforce planning; including resourcing, recruitment and retention, and ensuring that the training and development needs of our people and services are realised; through robust training and development programmes, both internally and externally.

What you'll be doing:



Leading and managing the nursing team to ensure outstanding patient care. Delivering evidence-based care for patients with acute and chronic conditions. Providing health promotion and preventative services, including immunisations, screenings, and travel health. Managing and developing chronic disease clinics (e.g. diabetes, asthma, COPD). Supporting minor surgery clinics, wound care, and a wide range of clinical procedures. Acting as a mentor and clinical supervisor for nursing colleagues and students. Taking a key role in quality assurance, compliance, and service development initiatives. Supporting the Practice in meeting its GMS and enhanced service contract obligations. Liaising with wider multi-disciplinary teams and external health partners.

What we're looking for:



NMC Registered Nurse with an adult nursing qualification. Minimum 5 years post-registration experience, including at least 2 years in general practice or community settings. Proven clinical competence in cervical cytology, immunisations, wound care, and managing chronic conditions such as Diabetes and COPD. Confidence in delivering evidence-based care plans for patients with long-term conditions. Strong leadership skills with the ability to support and develop team members. Effective communicator, comfortable influencing change and working across multidisciplinary teams. Teaching, mentoring, or clinical supervision experience.

Desirable (but not essential):



Independent prescribing qualification. Formal qualifications or training in Diabetes, COPD, asthma, family planning, CHD, or cytology. Mentorship or teaching qualification.
