Are you passionate about improving cardiovascular health and making a tangible difference in people's lives? We are seeking a dedicated Lipid Practitioner to lead efforts in identifying, managing, and treating hyperlipidaemia-including Familial Hypercholesterolaemia (FH)-within primary care.


In this vital role, you will work closely with primary care teams and specialist lipid services to support early detection and effective management of patients at risk of or living with cardiovascular disease (CVD). Using your clinical expertise and leadership, you'll deliver seamless care, provide education and support to healthcare professionals, and contribute to research and quality improvement initiatives.

Main duties, tasks & skills required

As a Lipid Practitioner, you will proactively identify and manage patients at risk of or living with cardiovascular disease, focusing on hyperlipidaemia and Familial Hypercholesterolaemia (FH). You will search primary care databases, contact patients for intervention, provide counselling, treatment, and follow-up, referring to specialist lipid clinics when needed. You will ensure lipid management aligns with NICE guidelines and support primary care teams through education and training.


Your role includes maintaining accurate clinical records, coding consultations to meet Quality Outcomes Framework standards, and fostering strong working relationships between practices and specialist services. You will work collaboratively across multidisciplinary teams, promoting integrated care and effective communication.


You will also participate in audit, research, and quality improvement projects, contributing to service evaluation and development. Leadership responsibilities include planning daily tasks, responding to queries, and supporting continuous quality improvement. Finally, you'll help identify cost-saving opportunities related to medication management while upholding high standards of patient care and professional development.
