The Role:

As our Live-in Care Coordinator, you'll be designated link between our clients, carers, and management. You'll ensure that care packages are well written, that carers are well-matched, schedules run smoothly, and both clients and carers feel supported.

What You'll Be Doing:



Contributing to the live-in care placements and ongoing rotas

Matching carers to clients based on needs and personality

Supporting carers with regular check-ins, problem-solving and unannounced spot checks

Building effective relationships with families and professionals to ensure continuity of care

Maintaining accurate records and updating care plans

Participating in the on-call rota (shared with the team)

Completing needs and risk assessments for new and existing clients.

Accurately record information on Careline Live, including evidence of conversation that have taken place.

What We're Looking For:



Experience in care coordination or scheduling (live-in care preferred)

Strong communication and people skills

Calm under pressure and great at juggling priorities

Experienced administrator and confident using care management software and Microsoft Office

A genuine passion for delivering high-quality care

Driving licence and access to a vehicle (preferred)

Experienced administrator

Job Types: Part time, Permanent

Pay: 25,000 pro rota

Additional pay: On Call payment of approx 157.50, one weekend per month
