Job Description

A Local Area Coordinator will support Adult Social Care teams by identifying resources, groups, services and organisations within the Cheshire East area that will benefit them. The service is available to all Adults with Care Act eligible needs following assessment. The Local Area Coordinator will coordinate information on available Community support within the Cheshire East Council (CEC) area and will inform community teams and the LiveWell site. A key role is to enable and support Adults with Care Act eligible needs, as well as those on the margins of eligibility to support the wellbeing and prevention areas of The Care Act and to access existing Community support rather than to rely on funded CEC Services.



Local Area Coordinators will also undertake assessments where required and identify low level equipment Assisted technology and access to Reablement services.

The Local Area Coordinators role is to work with people in the community who have Care Act eligible needs and assist them to source support within their local area.



The role involves working closely with Care Community colleagues in multi-disciplinary teams to ensure people have access to resources within their local community to assist them to access these easily.



You will need to have experience and knowledge of working with adults in the community and be able to work independently as well as coordinating your role with other Professionals. Knowledge of the local area would be of benefit although the role could cover anywhere within Cheshire East.



We are looking for someone who is enthusiastic, creative and someone who is able to identify needs quickly and get these into place. You will work within a team of other Local Area Coordinators and will work closely alongside Social Workers, Social Care Assessors and Occupational Therapists within your role.



Hours are 30 per week, ideally over 4 days to meet the needs of the service. We are currently using a hybrid model of working meaning that some time will be office based as well as working from home. Most of your work will be carrying out visits within the local area.



An enhanced DBS will be required for this role.
