An exciting opportunity has arisen to join our dynamic leadership team at Sirona Adult Learning Disability Health Service (ALDHS). We are a diverse multidisciplinary team providing health intervention for adults with learning disabilities in the community setting. As a larger service we are divided into locality multidisciplinary teams in Bristol, North Somerset and South Gloucestershire



We are seeking a highly motivated individual to jointly lead our Bristol locality team. You will work closely with other locality leads, professional leads and project lead as a key member of the ALDHS leadership team. This leadership model ensures that the clinical professions are well supported by clinical leads whilst the locality leads bring together the multidisciplinary team around the person, supporting complex cases, coordinating multi-profession involvement, safeguarding and supporting team wellbeing and cohesion. There are 2 locality leads based in Bristol due to the larger size of the team. They work closely with 2 others 1 each in North Somerset and South Gloucestershire.

Key Responsibilities:



Lead the locality team in Bristol working alongside 3 other locality leads across Bristol, North Somerset and South Gloucestershire.



Apply clinical skills to complex clinical situations to deliver coordinated and safe multidisciplinary working.



Liaison with health and social care agencies to facilitate safe care.



Promote the wellbeing of the team.



Leading the referrals triage and advice & guidance processes on a duty basis.



Implement and oversee quality improvement initiatives.



Collaborate with the leadership team in decision-making processes.
