We are seeking a skilled and dedicated Locum Advanced Clinical Practitioner (ACP) to join our supportive team at our GP Surgery in Plymouth. This role involves working 30 hours per week, focusing on managing minor illnesses, prescribing, and conducting home visits. You'll play a vital role in delivering high-quality primary care to our diverse patient community in a modern, well-equipped practice.
Key Responsibilities
Assess, diagnose, and treat patients presenting with minor illnesses
Prescribe medications in accordance with V300 prescribing qualifications
Conduct home visits to provide care for patients unable to attend the surgery
Collaborate with the multidisciplinary team to ensure seamless patient care
Maintain accurate and timely patient records in compliance with clinical and legal standards
Support safeguarding processes for adults and children
Participate in clinical audits and quality improvement initiatives
Escalate clinical concerns to the lead clinician as needed
Qualifications and Skills
Registered with the NMC (Nursing and Midwifery Council) or other relevant professional body (e.g., HCPC)
Advanced Clinical Practice qualification with V300 prescribing credentials
Proven experience in primary care, urgent care, or a similar setting managing minor illnesses
Competence in conducting home visits
Excellent clinical decision-making, communication, and interpersonal skills
Ability to work independently and collaboratively within a multidisciplinary team
Commitment to continued professional development
