About the Role

We're working with local authorities across

London

to fill essential roles in adult frontline services, including:

Hospital Discharge & Community Recovery Mental Health Services Adult MASH & Safeguarding Assessment & Review Teams Learning Disabilities & Autism Support

What You'll Be Doing

Depending on your skillset, your role may involve:

Managing caseloads across hospital discharge, community or safeguarding teams Undertaking Care Act assessments and Mental Capacity Act reviews Coordinating discharge planning in collaboration with NHS and MDTs Supporting adults with learning disabilities to achieve independence Working in multi-agency MASH teams to respond to safeguarding referrals Contributing to personalized care and support plans, reviews, and risk assessments

Social Work qualification

Social Work England

registration

Minimum 2 years

of experience in statutory adult social care A strengths-based, person-led practice approach Ability to work flexibly across your local London area (hybrid & in-person roles available)
