To work in support of registered nurse in the assessment, planning, implementation and evaluation of care and to work without direct supervision of a registered practitioner.

As part of a multidisciplinary team, the post holder will carry out specific care duties for patients delegated by a registered nurse or other relevant professional. On a daily basis they will be required to support the nurse(s) to supervise and deliver excellent standards of care which promotes privacy and dignity, whilst enhancing independence, rehabilitation and personal choice.

Key Responsibilities:

Be Champions the residents experience, encouraging good levels of feedback from residents, relatives, visiting professionals and colleagues to constantly improve quality of the service.

Champions independence and personal choice to ensure the highest possible standards of person-centred care.

Undertakes appropriate training to contribute to the development of self and colleagues.

Performs escort duties as requested to hospital and other professional visits.

Participates and supervises direct personal care always promoting privacy and dignity.

Proactively monitors all standards of care; undertakes designated audits including equipment audits, environmental audits, and care audits to ensure safe and effective care which meets required standards. Take corrective actions from audit findings in conjunction with nurse(s), Deputy Manager and Home Manager.

Supports nurses in leading the care team:

A good role model for colleagues promoting person centred care.

Help encourage team working throughout the Home.

Supports the nurse(s) in leading of the team and supervision of staff to ensure that shifts run smoothly, delegating as appropriate

Takes the lead with ensuring that relevant supplementary documentation is completed by others in the team.

Undertakes staff supervisions and appraisals as delegated by the Home Manager.

Actively leads the induction of new members of the care team to ensure adequate knowledge and skill of new team members, ensuring completion of induction programmes and supporting with Care Certificate.

Perform clinical skills for which they have been assessed as competent:

Will undertake clinical skills to include physical observations/vital signs, urinalysis, skin inspection, aseptic technique including simple wound dressings, blood glucose monitoring and gastrostomy Care and administration of gastrostomy tube feed
