Midlands Partnership University NHS Foundation Trust has an opportunity to join our Crisis Resolution and Home Treatment Service Team based in Stafford in South West Staffordshire.



The Crisis & Home Treatment Team, based in Stafford at St Georges Hospital, is a multi-disciplinary team which is operational 24/7, and provides a service to the population aged 16 and above residing in the South West Staffordshire Locality.



The team provide an urgent response and a comprehensive Mental Health Assessment to people who are presenting in an acute Mental Health Crisis, and Home Treatment to those people requiring a period of short-term intensive intervention.



Working in an identified adult community mental health pathway covering 24 hours per day, 365 days per year, with the flexibility of working across pathways, dependent on skill and service need.

Delivering a high quality clinical service to patients and their families comprising:



Accompanying Band 6 staff in the assessment of crisis situations

Assisting with the provision of effective home treatment to prevent hospital admission including effective physical health assessment and care



Working collaboratively with other Trust services to provide seamless care for service-users on defined care-pathways

Responsible for working in partnership with agencies external to the Trust supporting service-users access to services in the independent and non-statutory sector.

What is it like to be a Healthcare Support Worker within Mental Health Services at MPFT - YouTube



Under the direction of the senior health and social care staff in the team and/or the shift coordinator you will assist in delivering interventions with an allocated group of service users with mental health needs. The interventions will be focussed on assisting the person to identify and achieve their agreed goals of Recovery. These will include interventions related to; meaningful occupation, accessing employment, assessing and promoting physical health, social inclusion, activities of daily living and mental health promotion. You will also be involved in supporting the service user to complete appropriate outcome measure tools and participate in reviews of the service user's care and effectiveness of the interventions provided.



Please note this role is shift patterns covering 24/7, you will be required to work night shifts.
