An exciting opportunity has arisen to employ a Mental Health Practitioner within the Neighbourhood Mental Health Team for 30 hours a week. This role will focus on the delivery of population based mental health care for the community of Wyre Forest. You will be working alongside a team of highly specialist practitioners and have direct relationships with the professionals within the local Primary Care Networks (PCN). Main duties of the job
The main duties of a Mental Health Community Practitioner within our team includes (but not limited to):
Completing assessments on patients referred within the Wyre Forest locality. Referrals would typically come from GPs, however, may also come from internally within the trust or other agencies.
Holding a caseload of patients with a range of mental health illnesses and providing treatment to these individuals using evidence-based interventions as appropriate.
To develop care plans with patients and the MDT, to the utilise these care plans to support them in recovery.
To be able to risk assess and risk manage patients in response to their current needs.
To take part in the duty system and act appropriately when dealing with patients who are experiencing difficulties in the absence of their allocated worker.
Receive clinical and managerial supervision regularly, and to also supervise and support junior members within the team.
