We are seeking a compassionate, experienced Mental Health Navigator to join a multi-agency service in Kent and Medway that supports people with complex mental health needs.
This is an exciting opportunity to be part of a new, integrated model of care that bridges the gap between primary care (GPs) and community mental health services. The role focuses on providing person-centred, trauma-informed, and holistic support, ensuring that individuals can access the right help at the right time without unnecessary barriers or repetition.
You'll be one of the first people individuals meet when accessing mental health support, so it's vital that you bring empathy, strong communication skills, and a genuine commitment to improving people's experience of care.
Key Responsibilities
Conduct initial, person-centred assessments using Dialog and Dialog+ tools.
Co-create personalised support plans focused on individual needs, goals, and aspirations.
Work collaboratively within a multidisciplinary team to agree the best interventions and services for each person.
Deliver one-to-one, CBT-based psychological interventions (training provided).
Manage a caseload effectively, identifying when specialist input is needed.
Assess and manage risk and safeguarding issues, escalating appropriately.
Build strong relationships with local services and community assets to enhance client support.
Maintain accurate and timely records within shared systems.
About You
We're looking for someone who is passionate about improving mental health outcomes and can work collaboratively across professional boundaries.
Essential Skills & Experience
Significant experience working within community mental health or wellbeing settings.
Experience of supporting individuals with complex mental health needs through a case management approach.
Minimum Level 3 qualification (e.g. NVQ/SVQ) in Health and Social Care or related field.
Strong understanding of the social factors affecting mental health and knowledge of local support services.
Excellent interpersonal and communication skills with the ability to motivate and empower individuals.
Confident in identifying and managing safeguarding risks.
IT literate with accurate record-keeping skills.
Desirable
Additional training in areas such as safeguarding, motivational interviewing, or mental health awareness.
Experience in signposting and building community partnerships.
