: Locality Peer Support Worker Hours: 37.5 hours per week Salary: 24,500.00 - 25,500 plus mileage allowance We are a Peer Support organisation with a focus on improving mental health through community relationships. We promote both recovery from ill health for people with recurrent and enduring mental illness, early intervention and mental/physical wellbeing for those at risk of developing mental health issues or who struggle with common mental health conditions. As a Locality Peer Support Worker, you be fully integrated into Rotherham Doncaster and South Humberside (RDaSH) Primary Care hub. You will spend part of your working week in the Hub Team functioning as a full member of the team. The remaining hours you will be engaging with and helping people to develop and access support in their local community. This could be through the existing support available through People Focused Group and other organisations working in the community. It could mean the development of peer support group sessions and working with individuals to create their own recovery plan. You will provide support and recovery guidance to individuals experiencing common and severe mental health problems, using your own life experience of mental distress or as a carer and recovery as a template. Key Responsibilities: o To develop a rapport and a therapeutic relationship with people, within a relational focused recovery model that will be at the heart of this new approach. o To provide regular and practical support to people in developing and managing independence and maintaining dignity and self-respect. o To be a fully functioning member of the Community Mental Health Team, carrying caseload alongside other multi-disciplinary team members. o Plan support in association with other members of the multi-disciplinary team and external teams working with the person. o Ensuring assessment, support/care-plans and intervention are shared and agreed with relevant individuals and agencies o To attend training in trauma informed care alongside team members and use systemic and relational ways of working with people within a peer support role. A key emphasis is to enable people to assume agency in their recovery and assist them in a holistic way to better health and wellbeing globally. This means: Working with people /peer networks and cultivate local communities around them o Supporting people to gain access to resources to include benefits, welfare rights, settled housing, inclusion opportunities and health promotion. o Ensuring that people understand who to contact within the different agencies involved in the support plan including employment education and training, leisure and culture and faith and community engagement as indicated by individual need. o Providing support with daily living to empower people to live independent lives. o Supporting people to recognise good physical health as well as mental health and encourage them to engage in appropriate interventions such as physical activity and healthy eating. As a Peer Worker you will employ your own experience to work alongside participants in an equal partnership on their journey towards independent and fulfilling lives. Main Duties o To work with a varied caseload of people in one to one and group settings o To use personal life experience of mental health issues in an intentional and professional way to provide insight and motivational support in achieving recovery goals o To develop effective relationships with a range of external organisations and agencies o To promote the service and the peer support approach to communities and agencies as appropriate o To keep accurate and up to date records o To ensure quality standards are adhered and that audits are passed o To ensure all statutory responsibilities are followed and reported as required General work-related expectations o To work within both RDASH and People Focused Group values, ethos and vision o To work in accordance with all policies and procedures of both RDaSH and People o Focused Group, particularly (but not exclusively) Health and Safety; Information Governance and Safeguarding o To commit to own personal development and attend training or development activities as required o To work in accordance with all relevant legislation o To undergo regular supervision and at least an annual appraisal o To undertake any other duties as required, and as appropriate to the post o Person Specification: ESSENTIAL DESIRABLE Qualifications o Good standard of literacy and numeracy o Experience & Knowledge o Lived experience of mental health service use - statutory or voluntary sector - as a patient or a carer. o Experience of providing peer support to those o Knowledge of NHS mental health services. o Experience of working with people facing a range of barriers and social issues o Knowledge of mental health issues & conditions, and of the challenges and issues that are likely to affect those who experience such difficulties o Understanding of Peer Support and Personal Recovery approaches o Awareness of managing self-harm. o Awareness of Substance Misuse. o Safeguarding adults/children experience o Experience of working with individuals (1-2-1) and groups of people in different settings to help them achieve their goals. o Experience of working with a range of agencies and organisations to develop effective working relationships o Knowledge of the locality (community groups, services available as well as local demographics) Personal, Relational & Relevant Skills o Excellent communication skills both verbal and written o Ability to communicate complex information to other professionals o Ability to work well with a multi- disciplinary team. o Supports RDaSH mission and values. Ability to cope under pressure o Good time management.
