Job Description

To act as keyworker for patients providing a link to metastatic breast cancer MDT providing care for patients diagnosed with metastatic cancer liaising and planning care with secondary care teams, consultants, Macmillan, GPs and primary care health teams to establish integrated care. To provide specialist metastatic breast cancer nursing undertaking specialist assessment of patients nursing needs, plan, implement and evaluate care delivery using expert clinical knowledge. competency and clinical skills. Manage a patient caseload autonomously undertaking health/ physical assessments, requesting diagnostics and make treatment decisions on patient care under the indirect or direct supervision of the Consultant Radiologist.



Undertake Acute Oncology Triage by requesting and organising investigation for patients and reviewing results. Undertake non-medical prescribing within the clinic settings to support patient treatment pathways. Undertake pre-chemotherapy assessments providing information, advice and support for patients and continue to support following post chemo-therapy. Create and provide specialist information and support for breast care patients and act as single point of contact for patients throughout their cancer journey.



Act as patient advocate where relevant, facilitating informed discussions, concerning treatment options and ensuring informed consent. Be an active participate and quorate member in the MDT contribute effectively and advise the MDT and members of the wider health care community, to enable the provision of high-quality patient care. This will include carrying out diagnostic tests, giving advice on physical care, symptom control, support planning, social and psychological care, communication, resources and equipment. Undertake full Holistic Needs Assessment (HNA), evaluation and implementation of patients support plan.



To undertake detailed and specialist clinical and psychological assessment and analysis of patients on the metastatic breast cancer pathway and their carers both face to face and virtually (telephone/video call). To participate in inpatient reviews as required for patients requiring assessment and treatment plans. Support the patient and families through difficult and sensitive end of life issues, with the aim of promoting optimal quality of life. Using highly developed interpersonal and communication skills including telephone, emails, face to face with both staff and patients to communicate and receive specialist and sensitive information.



Managerial To lead and support on developing guidance and implementing new initiatives to improve the service in line with evidence based practice. Liaise with other members of the multi-disciplinary team to ensure seamless patient care through the development of protocols and pathways. Completes service developments/services with the support of the lead cancer nurse and consultant support. Work with the Multi-disciplinary team and service to ensure national standards are met.



Ensuring timely management and resolution of unexpected events and situations. Establish and maintain good working relationships within the multi-disciplinary team and cancer networks. Contribute to the development of guidelines, policy and procedures for the management of breast care patients. Maintain accurate patient records in line with NMC and submit information to relevant databases.



To support the process of Clinical Supervision within the Service. Understanding of safeguarding procedures and knowledgeable with respect to safeguarding triggers and risks. Education To identify, assess and meet the educational needs of the patient and their family/carers. To participate in educational programmes which promote the profile of the Trust, MacMillan Cancer Support and Oncology Breast Care Nursing Service.



Attend and participate in Breast Care Meetings, conferences and professional forums locally and nationally to maintain up-to date knowledge. To continue own professional development in line with NMC guidelines and NMC. To support to the team acting as an expert resource to both junior members of the breast care team and new staff within the service. Research To participate and complete audits and peer reviews.



Communicates with local and national research centres for appropriate trials. Maintain, advance and develop personal theoretical knowledge, high standards of clinical competence and professional development through self- education and attendance of relevant courses of study.
