The Overnight Mobile Support Worker will deliver high-quality, person-centred care and support to individuals residing within the Borough of Telford & Wrekin. This role forms part of the Planned Overnight Care service an innovative and highly utilised provision designed to enable individuals with assessed care and support needs to remain safely in their own homes and communities.
Key Responsibilities
Provide overnight domiciliary care services in accordance with the Care Act 2014, promoting wellbeing and prevention.
Work in pairs from a shared vehicle to deliver care across various households within the borough.
Respond to referrals efficiently, ensuring optimal use of service capacity and geographic coverage.
Deliver care between the hours of 10:00 PM and 7:00 AM
Support individuals with a wide range of care needs, including complex and short-term changes in condition.
Facilitate transitions from waking night services to Planned Overnight Care where appropriate.
Scope of Care and Support
Care duties may include, but are not limited to:
Personal care and hygiene assistance
Toileting support, including catheter management
Repositioning and physical transfers
Welfare checks and emotional reassurance
Medication administration
Assistance during illness or infection
Dressing changes and general overnight support
Communication with other professionals and care providers
Person Specification
Essential:
Previous experience in domiciliary or overnight care
Level 3 in Health & Social Care (or willingness to obtain)
Valid UK driving licence (Non - drivers may be considered based on skill set).
Ability to work flexible hours and travel across the borough
Strong interpersonal and communication skills
Commitment to safeguarding and promoting the wellbeing of service users
