Job Summary

We are seeking a compassionate and dedicated Community Care Worker to provide essential support to individuals in need within our community. The ideal candidate will assist clients during the night, ensuring their comfort, safety, and well-being. This role requires a strong commitment to delivering high-quality care while promoting independence and dignity for those we serve.

Duties

Assist clients with personal night tasks, including bathing, turning, nutrition and hydration needs and pop in visits during the night. Administer medication as prescribed and monitor clients for any side effects or changes in condition. Offer companionship and emotional support to clients, particularly those dealing with dementia or hospice care. Assist clients to transfers when necessary, particularly when assisting clients with mobility or transferring them safely. Maintain accurate records of care provided and report any concerns to the appropriate healthcare professionals.

Qualifications

Previous experience in a care role is essential; refresher training will be provided. Driving is essential Familiarity with dementia care principles and hospice care practices is desirable. Strong interpersonal skills with the ability to communicate effectively with clients, families, and healthcare professionals. A compassionate nature with a genuine desire to help others improve their quality of life.
