Jaffray Care are a charitable and specialist provider of care and support for adults.
We are looking for an experienced Care Practitioner to support our Service User's independence, rights and choices, to promote an play a vital role in providing direct support to the staff nurses at Lime Court.
The Night Care Practitioner will work as part of a team ensuring that all Service Users receive high quality care. They will assist and support the Service Users in a 'hands on' capacity with everyday activities based upon their individual needs and wishes.
The Care Practitioner will assist Service Users with their personal care, paying attention to the individual's care plans/guidelines. They will be expected to assist Service Users with their toiletry needs throughout the day according to their care plans.
The Night Care Practitioner will work under the guidance of the Nurse who will provide ongoing training and support as required. All night shifts are waking nights.
Care Practitioners have a responsibility for ensuring they fully understand and are able to deliver support appropriate to the individual.
Advanced MDS (Desirable or willing to undertake) bio dose e - learning, good IT skills and confidence in computer based work is desirable.
Understanding the legal differences between types of drugs and the legal frame work that underpins the management of medicines is desirable.
Administer, record , assist with monthly checking in of medications in accordance with the instructions of the prescriber as documented on the medication administration record (MAR)
Make a clear, accurate and immediate record of all medicines administered, intentionally withheld or refused by the service user.
The Night Care Practitioner is one of a team providing 24 hour/7 day week care to Service Users; this requires maximum flexibility which may include day cover.
You will be expected to carry out a detailed handover to staff at the start and end of each shift.
We will be looking for the following skills and qualifications:
Essential - QCF LEVEL 3 in Health and Social Care.
Effective communication and documentation essential.
Understand consent to care, DOLS, MCA, Care Act, Duty of candour, Mental Health Act, understanding of confidentiality and the principles of Data protection.
Experience of liaising with MDT professionals, other outside Professionals as well as next of kin and other people involved in the lives of our service user's.
Minimum ONE year's experience in long term care or assisted living of working with people who have Learning Disabilities, Mental Health issues, associated Health Issues and behaviour of concern.
Ability to handle multiple priorities, possess effective written and verbal communication skills, demonstrate good judgement, problem solving and decision making skills, ability to react calmly in emergency situations.
Ensure the dignity and privacy of the Service User is preserved when medicines are given. Report all concerns to the Nurse Care Manager, ensure medication is available, fit for use and is handled correctly. Assist in Medication Ordering Process as required.
Evidence compliance with medication policies and procedures by undertaking appropriate audits as requested.
