We have an exciting opportunity within our Virtual Hospital Service to join our lung health nursing team as a Nurse Adviser, where you will have the opportunity to contribute to delivering high quality clinical care to patients within the service by leading on defined aspects of care.
Our lung health nursing team are committed to comprehensively completing the targeted lung health check programme as per the standards of the National Protocol. Providing telephone consultations to pro-actively screen and identify high -risk patients for CT Lung assessment, for the early intervention of potential lung cancer.
As a Nurse Adviser in our service, you will work closely with the clinical services Manager to ensure that the service is safe, effective, and well-led to
ensure the delivery of a seamless service to our patients. This is a truly exciting opportunity where you will liaise with other hospital and community teams along a pathway to support the delivery of patient and family centered seamless care, as well as providing advice and education to patients and their carers.
Everyday you will...
Complete the LHC via telephone in line with the LHC(Lung Health Checks) Telephone Standard Operating Procedure undertaking LHC risk scores, and reviewing the inclusion and exclusion criteria for the programme with the patient.
Complete brief intervention smoking cessation and refer onto smoking cessation services where required
Be a defined point of contact for patients/carers for defined episodes/pathways of care and provide clinical care, specialist nursing advice and psychosocial support
Assess health and psychosocial needs of individual patients, and provide care to meet these needs, through a process of shared decision making
Deliver expert nursing care to patients in agreed practice settings, working with nursing/multidisciplinary teams.
Work closely with clinical and management teams in the delivery of key objectives

A valid NMC registration
Experience of working with respiratory conditions
Sound understanding of respiratory nursing standards and key performance indicator
Good at planning and organising with previous coaching/health promotion experience
