Nurse Associate up to 37.5 hrs /week
Salary: Between 26,208 and 27,924 per annum

We currently have a patient population of around 14,000 patients; with further growth is expected with new developments in the area. We have successfully increased our clinical team size to meet the growing demand to ensure that the future needs of our patients are met. We operate from two surgeries, one in Combe Down and one in Odd Down.
We are part of the Minerva PCN which encompasses around 38,000 patients spanning the southwest and northwest of Bath. We are excited by the changes occurring and the opportunities available within our PCN, which are enabling us to develop our service in line with the future direction of Primary Care.
We are looking to recruit a Nursing Associate to join our nursing team of 11 nurses of which 2 are nurse associates.
As a registered professional regulated by the Nursing and Midwifery Council (NMC), the Nursing Associate works within the NMC Code and Standards of Proficiency and contributes to safe, effective, and compassionate care within the practice.
The main duties of the role are as follows but is not exhaustive. Relevant training will be implemented as needed.
Deliver delegated care to patients in line with agreed and defined care plans, within individual scope of practice.
Contribute to the monitoring of patients with long-term conditions and support care that meets Quality and Outcomes Framework (QOF) targets.
Undertake clinical activities such as wound care, health monitoring, and other delegated procedures appropriate to competence and training.
Accurately document care delivered
Ensuring that samples are dispatched to the appropriate laboratories in a timely fashion
Ensuring that anomalies are reported to a senior member of staff
Notifying the GP/Nurse of any significant patient issues
Undertake basic patient observations to include blood pressure reading, weight and height
Monitoring, recording, ordering and re-stocking of clinical supplies and equipment as directed
Vaccine/cold chain storage and vaccine re-stocking and stock rotation
