An exciting opportunity has arisen for a full-time Band 4 Nurse Associate to join the York Community Response Team. This role offers a rewarding chance to take on a challenging and varied position, working primarily with our nursing team to provide nursing interventions within patients' home environments, with a keen focus on the 'home first' approach and preventing avoidable hospital admissions .



You'll be part of an experienced, friendly multi-disciplinary team in a supportive work environment, all while delivering high-quality care to our patients. Our well-established teams serve the historic city of York as well as the surrounding countryside in North Yorkshire. The team includes Physiotherapists, Occupational Therapists, Nurses, and Support Workers. Our team works in collaboration with Primary Care, York Frailty Hub and other partners to deliver Urgent Care Response and short-term reablement/rehabilitation programs.



Interviews will take place week commencing 21st July 2025.



The Nursing Associate will actively participate as part of the Multi-Disciplinary Team within York Community Response Team to provide a seamless service to patients with minimal supervision for a Registered Practitioner



The Nursing Associate will provide general and specific care for patients and will be competent to work with professional disciplines, working alongside Allied Health professionals to provide a home first approach to care provision.



The Nursing Associate will be required to play an integral role in the delivery of safe and high quality care, as delegated by the Registered Practitioner.



The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy. Access to a vehicle that can be used for business purposes and hold a full and valid driving licence (role will involve travel to numerous addresses that cannot be easily accessed by public transport means) is required.
