We are excited to introduce a new virtual ward service and remote monitoring service designed to deliver coordinated healthcare to patients in the comfort of their homes, replicating the support and monitoring traditionally provided in a hospital ward. As a Band 6 Nurse you will play a crucial role in this service, the nurses will be delivering care to high acuity adult NHS patients. These are patients with complex medical conditions for example Asthma, COPD, diabetes, frail elderly patients) who have been discharged from hospital and instead of being monitored in the ward, will be monitored at home. These patients will have the appropriate monitoring devices at home, and the nurse role is to carry out an initial triage, resolve where possible and escalate to the hospital team if they require further clinical reviews/intervention
Responsibilities
Patient Engagement and Assessment
: Provide education, support, and clinical assessment to patients within the virtual ward.
Vital Sign Monitoring
: Oversee and monitor patient vital signs (e.g., heart rate, blood pressure, temperature) that are automatically uploaded to our platform 3-4 times daily.
Clinical Decision Making:
Use clinical expertise to interpret vital signs, make informed decisions, and escalate care when necessary.
Patient Education
: Educate patients on managing their conditions and using remote monitoring equipment effectively.
Documentation:
Maintain accurate and timely records of patient interactions and clinical decisions.
Collaboration:
Work closely with a multidisciplinary team to provide coordinated and comprehensive care.
Essential Requirements
Must be a registered NMC nurse
Must have secondary care experience
Experienced in dealing with patients with respiratory conditions including asthma, COPD and pneumonia
Must have your own personal laptop to use
