Job Description

Provide a service that meet the needs of service users and their carers inclusive of all required patient records in order to provide high quality support services. Provide administration Clinical and dispensing of medication under the direction of medical and nursing professionals and/or manufacturers guidelines. Support nurses during medication round. Prepare for and join GP and psychiatrist reviews and district nurses on their visits under the direction of the Clinical Lead.

Document outcomes of visit to support the patient. Coordinate with family members regularly attending meetings as required and at times offering counselling and support to the patient. Support and enable colleagues to meet the needs of patients who express themselves through difficult or challenging behaviour. Ensure that these approaches are linked to nursing support plans.

Coach and mentor the support workers in order to support with the individual development of each support worker in the team. Support the Staff Nurse to ensure that the service meets all relevant quality standards, related to professional regulatory bodies. Undertake any reasonable duties/responsibilities to meet the needs of the organisation
