Job Overview



This is an exciting new post specifically developed to provide placed based care for the farming community working within Holsworthy Livestock Market. The role of the Nursing Associate/Nurse is to work seamlessly with the Primary Care Network and the Community Navigator plus other members of the multi-professional team associated with the Livestock Market Health Hub project. Providing and monitoring care and contributing to the ongoing assessment of farmers and their families, as agreed with supervising clinician(s) and in line with the service pathways. They will monitor the health needs of the farming community within their care in partnership with the multi professional team. They will provide coaching, motivation and education to patients to support them to manage their own health and wellbeing living independently and improving their health outcomes in conjunction with the Community Navigator. This post will be actively involved in research within the Farming Health Hub.

Key Responsibilities and Duties



Communication and Working relationships



Always act professionally in line with PCN code of conduct and communicate effectively with dignity and respect to all patients, relatives and others giving consideration to potential barriers to understanding.

Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility for care.

You will be able to assess clinical risk and know when to seek advice and escalate to the appropriate professional and able to communicate effectively with patients. You will also be able to carry out clinical tasks to a high level of competency, organizing and prioritizing your own workload.

Management and personal Development



You will be a registered Nursing Associate or Nurse, on the NMC register, with a Foundation Degree in Healthcare Practice (Nursing Associate) or an equivalent registration such as a higher national diploma who has worked within a multi-disciplinary team. Working in line with the values and behaviours of the NMC standards.

Comply with the NMC regulations relating to revalidation to maintain NMC registration.

Maintain a professional portfolio of skill development in line with NMC code of professional practice.

Undertake in service training relevant to the post.

Actively participate and attend team meetings.

Supervise and delegate appropriately to support staff within the team

Develop skills in relation to health and wellbeing coaching, personalized care and neighborhood teams.

Clinical Activities



As a Nursing Associate/nurse you will be working as part of the multi-disciplinary team providing high quality nursing care to patients and their families within the farming health hub.

You will be giving compassionate, holistic, evidence-based practice to patients using a range of interventions within the defined scope of the role and within PCN approved policies / procedures and individual care plans.

Have the ability to work without direct supervision, at times delivering care independently in line with the individuals plan of care.

Demonstrate sound knowledge of care and treatment in order to undertake non-complex nursing assessment/reassessment within the Livestock Market Health Hub under the direction/supervision of senior staff.

Perform, understand and report patient risk assessments, for example, nutritional, falls, pressure, ulcer assessments and long-term condition reviews.

Report back from the livestock market and share information with the registered practice on condition, behaviours, activity and responses of individuals.

Undertake a full range of clinical skills as per competency framework, including care wound, venipuncture, administration of medical with stable patients, peak flows, blood sugars, ECGs, performing spirometry

Coach and motivate patients with long term health conditions through multiple sessions to identify what is important to them in terms of their health and wellbeing. Supporting patients to self-identify goals using coaching and behaviour techniques.

Support to improve PCN health challenges, for example, in diabetes, weight management, smoking.

Engage in public health, community development, and in the reduction of health inequalities.

Strategic Development, Planning and Organising



Work in an effective and organised manner demonstrating excellent case load management.

Demonstrate the ability to organise structured clinics within the Livestock Market in partnership with Kivells and Imagine If, whilst remaining flexible in response to demand.

Working with the social prescribing service to connect them to community-based activities that support the patients' health and wellbeing.

Administrative



Ensure clear, concise, accurate and legible records are made to maintain professional standards of record keeping in line with the PCN policies.

To maintain compliance with the PCN and statutory responsibility for confidentiality

Ensure proper use of the PCN resources in the course of business, ensuring minimal waste and minimal cost.

IT Systems and Process



Ensure ability to manage electronic records system in line with PCN policy

Key Results/Outcomes



The key outcome is for patients to develop the knowledge, skills and confidence they need to self-manage their long-term health conditions and to work on the health and wellbeing related goals that are most important to them.

In addition, the nursing associate/nurse will support the engagement of patients in a wide range of healthy living programmes, peer support programmes and patient education programmes both within the PCN and in the wider community.

Line Management Responsibilities

Freedom to Act



The role will also deliver specialised behaviour change and health education interventions both on a 1:1 and group basis after completing PCN approved training. E.g. smoking cessation, healthy weight coaching, Type 2 Diabetes Structured Education Courses, HOPE Course, Pain Cafe etc

The post holder is required to comply with all relevant Code of Conducts for the role, including the PCNs code of conduct. All staff are required to support the PCNs commitment to developing and delivering excellent customer service by treating patients their carers, families, friends, visitors and staff with professionalism, dignity and respect. All staff are expected to behave in a professional manner and not to bring the Trust into disrepute.
