An exciting part time opportunity has arisen for an experienced and committed Nursing Support Worker to join the Community Nursing Team, working alongside the Community Diabetic Clinical Nurse Specialist within Carmarthenshire.
Working as part of the team, you will be responsible for providing high quality care to patients in their own homes as part of a nursing team, with the guidance and support of the Community Diabetic Clinical Nurse Specialist or designated representative.
The post holder will ensure high standards of individualised care are delivered to patients and their families and carers within the home environment, by following care plans and treatment interventions as delegated by the registered nurse, as well as having the ability to work competently without direct supervision. Community Nursing Teams have a wide-ranging nursing skill mix and pride themselves on supporting professional development and improving high standards of community practice in line with current health policy.
This is a permanent position worked over a 7-day period including weekends and bank holidays if required.
The post holder will need the ability to travel between sites in a timely manner.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
