We are looking for a skilled nurse to join our Rapid Response Team in Maidstone as Team Leader. Your role will involve:
Conducting comprehensive assessment, planning, implementation, and evaluation of holistic patient care.
Developing integrated care plans in collaboration with patients to ensure personalized and effective treatment.
Coordinating the ordering of necessary equipment and facilitate onward referrals as required.
Serve as a senior practitioner and lead, providing guidance to junior staff in conducting assessments and reassessments of service users and pathways with complex needs within the relevant field.
Actively contribute as a member of the multi-professional team to deliver high-quality patient care.
Perform a range of advanced clinical interventions, promptly identifying and addressing any deterioration in patients' conditions.
Managing a daily caseload of patients, focusing on those with complex needs.
Provide support in the duty role, including triage completion.
Assist the Clinical Lead in effectively managing the team.
Offering strategic recommendations to senior managers to prioritise initiatives, drive service improvements, and achieve business objectives and statutory requirements.
