About the Role

Join Wiltshire Council's dedicated

Reablement Team

and play a key role in helping individuals rebuild independence and confidence following hospital discharge. As an Occupational Therapist, you will support people to regain skills, reduce long-term care needs, and enhance their quality of life.

This role offers autonomy, varied caseloads, and the ability to truly shape outcomes for each person you work with.

Key Responsibilities

Complete

strengths-based assessments

and develop personalised reablement plans focused on independence and recovery. Provide

expert clinical advice

on equipment, adaptations, and decision-making to colleagues and service users. Train and support Reablement Team members to deliver safe, effective interventions. Conduct home visits and assessments in a range of settings, including hospitals. Manage your diary and workload independently while delivering high-quality, person-centred care. Work collaboratively with the

Principal Occupational Therapist

and wider multidisciplinary teams. Promote positive, safe, and sustainable outcomes for individuals following hospitalisation or periods of reduced independence.

What We're Looking For

A recognised

Occupational Therapy qualification

Minimum 2 years post-qualification experience

Valid

HCPC registration

Full

UK driving licence

(essential for community visits) Strong communication, assessment, and clinical reasoning skills Passion for reablement, independence, and strength-based practice

Working Pattern

09:00 - 17:00 Hybrid role:

50% of hours will be face-to-face

in clients' homes or community settings.

Pay Rates

(via agency):

PAYE:

from 27.92 per hour
Enhanced rates apply for Sundays, bank holidays, nights, and overtime (full rate table available on request).
