Are you a qualified Occupational Therapist seeking a new challenge in a crucial service?



We are currently seeking an Occupational Therapist who will work collaboratively with mental health Social Workers and partner agencies to meet the needs of service users with severe and enduring mental health problems. Professional Occupational Therapy assessment and intervention will be strengths-based to create opportunities, build resilience and long-term support networks so that people live their lives as successfully, independently, and safely as possible.



The post holder will be responsible for the review and assessment of individuals living in supported living placements or living independently with a package of care to determine if the level of support and home environment is conducive to physical and mental health.



You will work collaboratively with an MDT team within the requirements of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983 and any other relevant legislation, guidance, and codes of practice, providing personalised information and advice to individuals, families, and their Carers.



The post holder will be working under the direction of the Social Work Team Manager and the Professional Lead OT, and will be based in the central MH social work hub in Oxford. The post requires travel throughout Oxfordshire and beyond in order to meet the needs of service users placed out-of-county.



The responsibilities of the role include, but are not limited to:



To support individuals to move to appropriate living settings working in collaboration with clinical teams, families, and providers.



To undertake timely and proportionate assessments, reviews and professional interventions and other duties relating to individuals, families, and carers.



Responsible for contributing Occupational Therapy assessment and interventions to support plans to ensure they are reflective of the persons needs and outcomes by providing a clear breakdown of how these are being met, regardless of determination of eligibility.



Identify and make necessary arrangements where an individual would benefit from services which will improve and maintain independence, reablement services and equipment.



To undertake face to face visits in the community and be responsible for the management and prioritisation of a reviews.



To assess and manage risk of patients and their families and to escalate concerns about unmanaged risk to clinical and operational leaders as required.



Take assertive action to prevent harm to self and others, and actively engage with both child and adult Safeguarding processes if appropriate.
