The focus of the service is to support community reablement, prevent unnecessary admissions to hospital or long-term care, and facilitate timely discharge from hospital.


The post holder will work in an Integrated Multidisciplinary Team with Health and Social Care colleagues.


The service is provided to people over the age of 18 years residing within the City & County of Swansea. Assessments and interventions are completed within a wide variety of settings; peoples' homes, nursing & residential care facilities, residential and community hospital reablement beds and In-reach across acute hospital sites.



The service also accepts referrals for children who may require adaptations to their homes following OT assessment via Disabled Facilities Grants.


The Occupational Therapy Team are present at the Common Access Point for Health and Social Care, contributing to the multidisciplinary screening of incoming referrals and carry a complex caseload of crisis and urgent referrals. The team are also an integral part of the Home First MDT screening; providing reablement to people discharged from hospital.


You will be expected to hold a varied caseload and work as part of a multi-disciplinary team to support people to maximize independence. You will carry out functional, holistic, and specialised assessments, and interventions. Interventions may include making recommendations for minor and major adaptations to client's homes.


You will be expected to supervise junior staff (Health/Local Authority) and support student placements.


You will participate in audits and contribute to service development, maintain professional standards and governance.


The successful applicant will be required to travel in a timely manner around the community and participate in a seven-day working roster.


The applicant should be highly motivated, enthusiastic, flexible and resilient.



The ability to speak Welsh is desirable for this is post; English and/or Welsh speakers are equally welcome to apply.
