Job Description

Ensure and promote excellent communication with all members of the MDT. Provide condition related information to children and their families or carers in clinic setting and at the bedside (pre-op and post-op), to include gaining informed consent, requiring empathy and reassurance. Set up and lead once-monthly patient groups within the community, providing information, support etc. Attend weekly clinical meetings as needed to foster collaborative MDT working.



Ensure confidentiality and adherence to data protection legislation. Ensure that a full and accurate record of care is documented in a timely manner utilising appropriate nursing and medical systems as well as our bespoke stoma care database. Ensure stoma care database and diary are used effectively and accurately. Promote inclusiveness and treat people with kindness and compassion.



Promote a team ethos and a positive working environment Ensure grievances/disputes/complaints are investigated thoroughly in a timely manner. Service Delivery and Improvement Influence, plan, organise and implement programmes of individual, core and strategic paediatric stoma care service delivery in accordance with Trust priorities. Ensuring clinical practices have children, young people (CYP) and their families and carers at the centre of service considering the children and young peoples lifestyle, gender and cultural background. Make decisions on the day-to-day management of service provision independently within competence boundaries to ensure patients pathway is adhered to.



Make autonomous decisions about patient care on a daily basis, considering best practice guidance, past medical history, along with recent and ongoing stoma requirements to make those decisions, some of which is likely to be conflicting. To facilitate delivery of a high-quality, cost-effective specialist service through monitoring and audit. Exploit opportunities to enhance childrens and their families/carers ongoing care through nurse led initiatives, audit and research. Organise and manage stoma care teaching and training as needed, usually on a weekly basis.



Liaise with representatives of ostomy manufacturing companies as required. Lead in ordering and utilising paediatric stoma care supplies in a cost effective and waste reduction manner. Set up and support a Paediatric Stoma Care Patient and Family Support Group. To utilise information systems to aid regular audit of clinical role and activity.



To monitor and evaluate stoma care appliances and equipment through involvement in clinical product evaluation. To participate in the Clinical Governance following Trust guidelines. To set, monitor and review paediatric stoma care standards of care. May participate in multi professional research and in national and local research projects and to seek publication of own work.



Presenting research findings at conferences infrequently. To develop, provide and update appropriate patient/carer written information and advice for service users. To develop protocols for paediatric stoma care as required, for example the Management Protocol for High Output Ileostomy. Such protocols are to be utilised across the organisation, and on occasion across the ICB.
