JOB DESCRIPTION Job responsibilities The post holder will be an experienced clinician, who, acting within their professional boundaries will provide clinical assessment and management of patients within practice. The post holder will support the delivery of high-quality care working within the Practice team, and with partners. Committed to the development of innovative practice and expanding evidence-based care which will contribute to the enhancement of the paramedic role. You will be expected to provide and maintain a high standard of care for patients as well as providing support to the GPs and other members of the primary health care team.



Supported by a skill mixed team, they will work within the practice, as an autonomous, accountable Paramedic, in the provision of a holistic approach for patients including assessment, diagnosis and treatment, to deliver quality patient services. Paramedics work autonomously within the community, using their enhanced clinical assessment and treatment skills to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to, for example, minor illness or injury, abdominal pains, chest pains and headaches. They are health professionals who have the capability to make sound judgements in the absence of full information and to manage varying degrees of risk when there are complex, competing or ambiguous information or uncertainty. The following sets out the key role responsibilities for paramedics: a.



They will assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the primary care team. b. They will advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services. c.

They will be able to:

o perform specialist health checks and reviews; o perform and interpret ECGs; o perform investigatory procedures as required, and; o undertake the collection of pathological specimens including intravenous blood samples, swabs etc. o perform investigatory procedures needed by patients and those requested by the GPs d. They will support the delivery of anticipatory care plans and lead certain community services e. They will provide an alternative model to urgent and same day home visits for the network and undertake clinical audits f.



The will communicate at all levels across organisations ensuring that an effective, patient centred service is delivered g. They will communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required h. They will maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice i. Prescribe/issue medications as appropriate or be willing to undertake Non medical prescriber training, following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways j.



Enhance own performance through continuous professional development, imparting own knowledge and behaviours to meet the needs of the service. Team Working Understand own role and scope in the organisation and identify how this may develop over time. Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working. Where involved in delegation of tasks, delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence of those taking on delegated duties.



Ensure clear understanding and utilisation of referral mechanisms within the practice. Prioritise own workload and ensure effective time-management strategies are embedded in own practice. General Duties and Responsibilities Enhance own performance through continuously developing own knowledge, skills and behaviours to meet the current and future requirements of the job and respond to the learning needs of the organisation. Act within acceptable parameters as an employee for the organisation, having regard to the applicable Code of Conduct for your role and ensuring own practice is in accordance with organisational policies.



Maintain own CPD and contribute to own personal development by participating in annual appraisal with line manager, developing a PDP, and actively participating in agreed learning activities and evaluating effectiveness of learning in relation to role. Regularly review own practices and makes changes in accordance with current and/or best practice, makes suggestions for improved practice and identifies where other activities affect own practice. To achieve and demonstrate agreed standards of personal and professional development within agreed timescales. To contribute positively to the effectiveness and efficiency of the service in which they work.
