Summary
We are looking for a Care Coordinator to join our team in the UK. In this role, you will play a vital part in ensuring the delivery of high-quality care services, contributing to our mission of enhancing the well-being of our clients.
Responsibilities
Coordinate care services for clients, ensuring their needs are met effectively.
Liaise with healthcare professionals and families to facilitate communication.
Maintain accurate records and documentation related to client care.
Monitor service delivery and ensure compliance with quality standards.
Assist in training and supporting staff members as needed.
Respond promptly to client inquiries and concerns.
Participate in care planning meetings to discuss client progress.
Promote a positive environment that encourages client engagement.
Qualifications
Proven experience in a care coordination or similar role is preferred.
Strong organizational skills and attention to detail.
Excellent communication and interpersonal abilities.
Ability to work independently as well as part of a team.
Proficiency in office software applications is desirable.
