We are looking for a compassionate, motivated Assistant Practitioner - Pathway Facilitator to join our evolving Community Place based team - Frailty Service.



The role of a Pathway Facilitator will provide exciting opportunity to work in a rewarding role, where the focus will be on delivering high - quality holistic care that keeps the Walsall population safe, well and supported within their own homes.

This post involves working within the Community nursing locality teams, supporting both District nurses, Frailty case managers and Specialist nurses, it involves t he proactive monitoring of patients post discharge from hospital, in order to promote and sustain their independence in the community setting and prevent avoidable hospital re-admissions.



Under the guidance of senior clinicians the post holder will undertake clinical procedures and delegated duties unsupervised to clients within their own homes, Clinics or care home environment.



Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 284,300. It provides inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor houses the full range of district general hospital services under one roof. The 170 million development was completed in 2010, and ongoing upgrades ensure the Trust now has state-of-the-art facilities, including a Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit. A new Urgent and Emergency Care Centre, opened in March 2023, has significantly improved emergency care facilities and capacity, providing almost 5,000 square metres of additional clinical space.

Key Responsibilities



Use skills and knowledge to support the needs of the patients in their own homes within 24 hours of hospital discharge. Including observations, medication checks, skin checks. To support short and long term care needs for patients in order to prevent unnecessary re-admissions to hospital. Able to effectively communicate at all levels of the organisation; to a variety of health professionals; users and carers, to provide the best outcomes Able to provide the interface between hospital and Primary, Community & Social Care settings. Able to challenge systems, professionals and decisions and effectively communicate the rationale for such challenges. Able to keep accurate contemporaneous documentation and care/ plans To support with the reduction in avoidable hospital admissions To use electronic alerts/ databases (CANAR, whiteboards) to assist with communication. To have a close collaborative working relationship with the high intensity user team. * To assess risks to patients which need to be managed in relation to their health and well-being.
