Job Description

The patient care coordinator role at Sur Wellness is one that requires you to assist and manage each patient's journey ensuring their health and treatment needs are met whilst also maintaining effective communication between the patient and their treating clinician. You will be required to identify each patient and refer on / or book in with the correct consultant based on their needs. At Sur, aftercare and follow up is as every bit as important to that of the initial consultation and treatment itself and therefore, you would be required to proactively keep patients engaged, noting all feedback as they move through their journey.

Our Patient Care Coordinator should possess skills able to understand and then explain often in layman's terms medical procedures and treatment plans to patients demonstrating an empathetic nature to sometimes sensitive medical information and diagnosis. You will be expected to work using your own initiative however work well within a diverse team of mixed professionals and staff at Sur. There will be times where patients can display challenging or emotional behaviours and it would be expected that you possess the necessary attributes in order to manage this effectively.

A coordinator would be expected to effectively manage workflow and dedicate suitable timelines for the entirety of each patient journey, this being from the first point of contact to final discharge. This often involves chaperoning patients whilst being consulted. This is a vital part of the role as you can then fully understand what has been discussed at consultation enabling you to effectively communicate this to the patient in future conversations as well as pricing the surgery or treatment itself.

At Sur we treat every patient from a 360 holistic approach and therefore, we want to create an environment that promotes a seamless referral system amongst a very skilled and diverse Multi-Disciplinary team. The patient coordinator role is one that is at the centre of this. Your relationship with each and every patient will ensure the referral process is as smooth as possible avoiding any inconvenience to the patient as best you can.

Job Type: Full-time

Pay: Up to 32,000.00 per year
