Job Description

Role Overview The post holder is to standardise the management of prescriptions by becoming a single point of contact for medication and prescription requests and queries. This will improve medicines safety by preventing errors, reducing waste and containing costs. The post holder will work as part of a multi-disciplinary team signposting requests and responses to pharmacists, receptionists, GPs and ANPs, to improve patient experiences and health outcomes also improving satisfaction for staff by reducing workload. Primary Duties and Responsibilities Action system online, and NHS app prescription requests sent online directly from patients within 1 working day after 6pm Action handwritten paper prescription requests forwarded on to us via reception within 2 working day after 6pm Action medication requests and queries from pharmacies sent via the practice individual NHS mails within 1 working day after 6pm Action external medication requests and queries from care home sent via the practice individual NHS mails within 1 working day after 6pm Action paper, postal requests from appliance contractors Deciding which types of review are needed (of the 19+) for a patient and when they are required Send appropriate booking links for review when there is a need for an appointment Knowing the different elements required for disease reviews e.g., which blood tests, whether BP and urine are necessary Signposting any acute requests to the appropriate group of clinicians including relevant information required to issue or decline the request if appropriate Electronic repeat dispensing repeat medications for patients that meet a specific criteria to reduce medication requests from monthly/bi-monthly to yearly/bi-yearly Action early requests for reasons such as holidays etc.



knowing where more information is required Issuing clinician only issue set medications for patients that are only allowed to have their medications on the same day Recognising when a patient may be overusing a medication and may require monitoring Issue 12 weekly dosette prescriptions to pharmacies for all Affinity sites when due so the patient nor pharmacy need to order these Action any medication changes required for dosette boxes, informing all necessary parties (the patient/pharmacy) within a timely manner, issuing any prescription required to fulfil the required boxes. Issue 3 monthly regular prescriptions to pharmacies for care home residents so the staff nor pharmacy need to order these Cleaning up repeat medication lists making sure issue durations are correct, removing any medications not used and requesting medications issued by other places are put into other medication etc. Liaise professionally and pleasantly with patients regarding any medication queries they may have. Learning clinical knowledge e.g.



knowing what medications are for what illnesses/diseases, what hormones HRT is composed of and subsequently what is needed based on reproductive organs Knowing the different elements/parts to diabetic materials and machines to understand better what the patient needs Knowing the different elements/parts to appliance materials and catheters to understand better what the patient needs Knowing alternative names of the same medication Meeting goals in the amount of review appointments booked
