Job Description

We have a fantastic opportunity for a dynamic and forward-thinking professional with broad experience in Patient Safety. We're seeking a strategic and innovative leader to collaborate closely with our Divisional Governance teams, helping to embed and sustain both the Patient Safety Incident Response Framework (PSIRF) and the associated Response Plan (PSIRP).



The ideal candidate will bring senior-level leadership experience, strong visibility across the organisation, and the ability to inspire and engage teams to foster a culture of safety and continuous learning.



With a solid track record in patient safety investigations and learning processes, including the development of PSII reports and After-Action Reviews, you will bring a well-rounded skillset to support the organisation's efforts to drive improvement and meaningful change.



As the Patient Safety Investigation lead, you will take the lead in shaping and maintaining a culture of safety through the ongoing coordination, implementation, and enhancement of our patient safety strategies. You'll work collaboratively with both internal and external partners to carry out thorough investigations. This involves gathering and analysing information from clinical records, engaging with individuals, and reviewing relevant policies, best practices, and guidance. The aim is to produce well-structured, high-quality reports within set timelines.



You'll need to quickly assess complex clinical scenarios, make sound, risk-informed decisions, and present your findings through clear recommendations and improvement plans. These will be shared with various stakeholders, including families, staff, commissioners, and coroners.



Beyond investigations, you'll also support the design and roll-out of improvement initiatives, help embed learning across the Trust and provide expert support in key areas such as human factors, Just Culture, the duty of candour, and investigation best practices.
