An exciting opportunity has arisen within the Clinical Governance Team for a Patient Safety Investigation (PSI) Lead. The post holder will lead Patient Safety Incident Investigations and other learning responses under PSIRF. Responsibilities include overseeing investigations, making recommendations, ensuring practice changes, and supporting Patient Safety Investigators. The role involves engaging with staff across the organisation, including Safeguarding Divisional leadership, Executives, and the Patient Safety Specialist, with a focus on openness and transparency.
This role is crucial for ensuring the Trust responds to patient safety incidents proportionately and constructively, emphasising continuous learning and meaningful engagement. Sussex Partnership NHS Foundation Trust operates within a governance framework that integrates professional leadership and operational management. The post holder will also analyse incident data to highlight trends and areas for action.
In order to be a successful Patient Safety Investigation (PSI) Lead in our team you will:
Hold a clinical qualification.
Be responsible for investigating PSIIs and supporting other learning responses with the Divisions, overseeing the reporting process ensuring that the Trust meets the quality and performance standards.
Provide recommendations and advice for future safety, treatment, care and well-being, in accordance with NICE and other relevant clinical and professional guidelines and best practice, of people who use services, their families, carers and staff.
Maintain records and data undertaking analysis and provide reports.
Ensure that all documents / case files pertaining to learning responses are secured in a safe and confidential manner.
Manage sensitive and contentious investigations dealing with highly emotional situations with Service Users, carers and families, staff and partner agencies.
Communicate with a range of internal and external stakeholders
