As Patient Service Co-ordinator you will be responsible for managing referrals from our service users, who are all those affected by kidney disease in Wales, including children, youths, adults, and families. You will build and maintain constructive working relationships with people who use our services, people with kidney disease, carers, colleagues, and community networks
Our Patient Service Co-ordinator acts as a vital liaison, responsible for managing all referrals from our service users, identifying the support needed, facilitating appointments for peer support and counselling, communicating between patients, referrers, volunteer peer mentors and counsellors. Referring patients to internal services as well as external organisations. Other duties include providing general advice and guidance, developing links with external providers, organisation or individuals that can support our kidney community of Wales, administrative support include assisting accessing healthcare services, maintaining referral records, and resolving referral concerns to support efficient and effective care.
As Patient Services Co-ordinator you will also build relationships with Health Care Professionals to help raise awareness of our services, building our referral partners to ensure all patients are supported at the point of need.
Salary is Grade 1 to Grade 2, based on a 40 hour work week with paid breaks. It is also based on experience.
