Torridge Health PCN looks after a population of 55,000 people we are committed to improving and improving and supporting our population's health by working with partner organisations and providing a person-centred care approach for all patients.

Our ethos is to look at improving health and to identify gaps in local health care provision and to work with care sector partners to support them. Our intention is not to duplicate services already implemented but to underpin and expand on the vital work that Primary, Secondary, Community and VCSP's already provide, thus ensuring sustainable health care services for the future.

Torridge PCN is looking to increase the Care Co-Ordinator role. The post holder will work alongside clinicians and other staff across the Torridge area and as part of a multi-disciplinary team.

The overall objective of the role is to help patients access the right services first time.

Main duties will include

Providing a point of contact for both staff and patients in coordinating care and will help to deal with incoming queries Identifying patients from a task list to arrange follow-up appointments with the most appropriate clinician, liaising with our clinical teams or other outside agencies to implement treatment plans and ensure appropriate services to maximise the health and quality of life for the patients. The initial focus will be on supporting outpatients who require ongoing support Providing administrative support to support the delivery of clinical work streams Collating monthly patient utilisation data Ensuring the directory of services remains up to date

Job responsibilities

Run Audits and searches where necessary to identify patients for review. Serve as the point of contact for practice staff, providers, care teams, family/caregivers and community resources where indicated, responding with empathy and respect. Help people manage their care needs by answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information relating to their care. Help develop the PCN social media platforms. Refer patients to the appropriate team member and make referrals on behalf of the team. Monitor referrals to ensure tasks are completed and care delivered by keeping in regular telephone contact. Signpost patients to appropriate community or local services. Support the PCN to deliver quality metrics, such as QOF, KPIs and locally commissioned enhanced services. Manage and review DPIAs, ensuring compliance with data protection regulations. Contribute to audits, reporting, and continuous improvement initiatives. Where necessary, attend multi-disciplinary team meetings. Document and monitor aspects of patient coordination and service delivery, supporting data collection and audit using the practices clinical system. Directly reportable to Ops/Line manager and Clinical Directors. Any responsibility identified during the course of the job. This is an evolving role, and the candidate must be flexible and adaptable.

About The Candidate



This is a new role that will continue to evolve within the PCN, and it requires a dedicated self-starter, enthusiastic about working with Practices at scale. You will need to have the following skills/qualifications:

Proven experience in primary care or within a Primary Care Network (PCN). Strong working knowledge of SystmOne clinical system. Excellent IT Skills inc. Word and Excel. Excellent communication, organisational, and the ability to work well in a team; however, also able to work independently without supervision and manage multiple priorities effectively. You will be able to review and improve working practices with shared experience across the PCN You will be motivated by helping people with care and empathy In delivering person-centred support in a non-judgmental way
Job Type: Part-time

Pay: 13.25 per hour

Expected hours: 30 per week
